Sunflower Communities has both assisted living and secure memory care openings at both Nagel Assisted living & Memory Care located in downtown Waconia and at Elmore Assisted Living & Memory Care located in Elmore, MN. Both of our communities have a mental health focus and we do not shy away from complex care to include two-person transfers and complex (non-physically aggressive) behaviors. Not only do we work with private pay individuals but also CADI, EW and TBI Waivers as well as GRH, Jarvis Orders, Civil Commitments and Rep Payees. Other community highlights include; one does not need to be 55+, complimentary furniture to include a bed, dresser, nightstand and flat screen TV, outdoor smoking areas and pets welcome at our Elmore Community. If you would like to make a referral, please email or fax me the individuals History & Physical document, a face sheet, current medication list and if applicable, nursing notes. Once received my colleague Nancy will reach out to schedule the assessment.
