We currently have assisted living and (secure) memory care openings.  Currently we have two communities: Nagel Assisted Living & Memory Care which located just West of the Twin Cities in downtown Waconia and walking distance to the park, lake, a move theater, coffee shops, restaurants, boutiques, etc.  Our second community, Elmore Assisted Living & Memory Care is located in Elmore, MN.  Elmore is a small rural community located about 40 minutes South of Mankato, MN. 

Both of our communities have a Mental Health Focus and we don’t shy away from individuals with high care needs to include complex (non-physically aggressive) behaviors.  We work with private pay individuals as well as those on CADI, EW and BI Waivers. We also work with GRH, Jarvis Orders, Civil Commitments and Rep Payees. 

Other highlights include; we have outdoor smoking areas, on-site OT/PT clinics, complimentary furniture and our community in Elmore allows pets (dogs and cats).

If you have a client needing immediate or future placement, please email or fax their History & Physical document (H&P), a current medication list, a face sheet and nursing notes (if applicable).
