 Sunflower Communities; Nagel Assisted Living & Memory Care and Elmore Assisted Living & Memory Care currently has openings. We offer compassionate, resident-centered living with a focus on mental-health. Both communities accept private pay as well as CADI, EW and TBI Waivers. We also work with GRH, Jarvis Orders, Civil Commitments and Rep Payees.

Other highlights include; complimentary furniture, on-site OT/PT and primary care services, outdoor smoking areas and our community in Elmore accepts pets. If you know someone who may benefit from our care, or if you'd like to learn more, feel free to reach out to me anytime.
